Quito Health: Terms

1. Who we are

This service is operated by Quito Health (a DBA of 3Ps Lab LLC) [legal entity details and postal address - LAWYER]. Contact: support@quitohealth.com [mailbox to be created; who answers it - AJ].

The service is a beta. It is offered by invitation only, in the United States only, to people aged 18 or older.

2. What the service does

• With the patient's permission, given on their health system's own sign-in page, it reads the patient's medication orders and related records (see the Privacy notice for the exact list). It never writes anything to the health record.
• It shows the changes it finds to the family's admin. Nothing reaches the family's wall screen until the admin reviews it and publishes it.
• The wall screen shows the list the admin published, the family's photos and the ticks made on it.

3. What the service does not do

• It is not medical advice and not a medical device. It does not diagnose, screen for, detect or assess any condition.
• It never recommends starting, stopping or changing a medicine or a dose. It shows the instructions the care team recorded, as reviewed by the family's admin.
• It can be incomplete, late or wrong. Always follow the patient's care team. When the screen and the care team disagree, the care team is right. Questions about medicines belong with the doctors and the pharmacist.
• It is not for emergencies. In an emergency, call 911.

4. Families, admins and members

• Each family has exactly one admin, who reviews and publishes, connects the health system, pairs the wall screen and invites other members. Other members can see the family's pages but change nothing. The patient, if they have their own sign-in, can also choose who the admin is.
• Caregivers: by agreeing to these terms you confirm that you are the patient, or that the patient (or their legal representative) has allowed you to help manage their medicine information. The patient authorizes access to their records on the health system's own page; they can withdraw it there at any time.
• Each member agrees to these terms for each family they belong to. When these terms change, we ask again before you can open the family.

5. Acceptable use

• Use the service only for a family you are part of, with the patient's permission.
• Do not try to reach another family's information, test the service's security without our written permission, or share your sign-in or an invitation link with someone it was not meant for.

6. Ending

• Any member can leave a family at any time. The admin can delete all of the family's information, or close the family, at any time (see the Privacy notice).
• We may remove an account or a family that breaks these terms, and the beta may stop [notice period and how families are told - LAWYER].
